It's an internal conflict because he often fights against himself, like the whipping, because he doesn't accept who he is.
The fact that John is an outcast in both societies, the reservation and world state, and that he struggles with the conflicting beliefs between the two is both an internal and external conflict. John help drives the plot of the story by causing conflict, like he did when he threw the soma out of the window, angering the crowd and causing a riot. John also ends up whipping Lenina when she arrives. He tries very hard deal with the reality of the "Brave New World" yet he inevitably goes insane and hangs himself.
John is struggling with the way of life in the new world and his mother's death in chapter 14 sends him spiraling down a path to madness. This is shown while he's at the lighthouse, when he whips himself.
